An advance decision (sometimes called an ADRT or ‘living will’) lets you refuse specific medical treatments in future if you lose capacity — and in England & Wales it can be legally binding if done properly. An advance statement is a broader, non‑binding record of your wishes, values and preferences about care. Both help people and professionals know what you would want.
This guide explains the facts clearly, gives practical tips, and points you to good, official places to get forms and further help.
